Primary Responsibilities

  • Patient Engagement & Scheduling:
    • Facilitate patient agreements
    • Schedule and coordinate telehealth appointments with patients, caregivers, and ancillary support staff.
    • Provide compassionate, professional communication to patients and families
    • Verify patient insurance eligibility and benefits
  • Care Coordination:
    • Facilitate communication between providers, patients, and external healthcare entities
    • Manage communications within an electronic medical record and route communications to appropriate team members
    • Manage new patient information including referrals, creating patient profiles, and verifying accurate information to support ongoing care
    • Ensure continuity of care and efficient workflow
  • Clinical Support:
    • Assist Head of operations in gathering credentialing documents
    • Assist in the setup of equipment for onboarding new clinical staff members
    • Gather necessary documentation of competencies and immunizations prior to onboarding new clinical staff
    • Manage and organize patient records in the EHR system
    • Support clinical team with administrative and operational tasks
  • Additional clinical and administrative tasks assigned

Required Skills / Abilities

  • Proven experience in a healthcare support role (coordination, or clinical office work)
  • Proficient in Microsoft Excel, Word, PowerPoint, and Google Drive/Workspace
  • High level of computer literacy and ability to learn new software and systems quickly
  • Exceptional organizational skills and attention to detail
  • Strong communication skills—both written and verbal
  • Ability to multitask and prioritize in a fast-paced environment
  • A true team player with a proactive, solution-oriented mindset

Preferred Skills / Abilities

  • Experience with Electronic Health Records (EHR) systems
  • Familiarity with HIPAA regulations and healthcare compliance
  • Background in supporting home health, hospice, or outpatient clinical teams
  • Minimum of 3 years of experience with key responsibilities
