At Pine Park Health of Reno, we're expanding our model for caring for seniors that provides high-quality, comprehensive in-home healthcare services to seniors all across Northern Nevada. From private residences and assisted living facilities, our team of nurses, physicians, advanced practice providers, and care providers visit patients where they’re most comfortable—creating a seamless, convenient, and personalized process for seniors and their caretakers.
The Role
The Nurse Practitioner is responsible for the diagnostic evaluation, management, and therapeutic health care of primary care patients of PPH residing in assisted living communities, residential care homes and private residences. This care includes-
- Obtain history and physical on new patients
- Order/interpret diagnostic tests
- Diagnose and treat chronic conditions and episodic illness
- Perform Advance Care Planning
- Accurate and timely documentation in the EHR
- Coordinates with other members of the care team, both internal and external partners
In addition to the above, the Nurse Practitioner is responsible for the annual comprehensive health assessments in accordance with health plan partners and Medicare guidelines. The assessments may be done in the home or clinic setting. This service includes:
- Conducts Health Assessment, including past medical history, review of systems, medication reconciliation, and physical exam
- Review and discuss the need to close care gaps and quality measures
- Perform/review standardized screening exams
- Identify all existing medical conditions
- Accurately and timely documentation of assessment and conditions to include quality MEAT Statements
- Identifies patients in need of chronic care management
- Identifies patients in need of pharmacy assistance
- Document Elements of Personal Prevention Plan and health issues to follow up with PCP
The Nurse Practitioner is also responsible for ensuring a safe and smooth transition of patients following an acute or skilled stay or ED visit. The care included is-
- In-home post-acute visit including history and physical and medication reconciliation
- Short-term management of episodic illness related to acute or skilled admission
- Care coordination with team members for the implementation of a safe discharge plan
- Identify barriers/obstacles to safe, self-management in the community and work with care team to overcome or identify alternatives
What We’re Looking For
- Current Nevada PA or APRN license, credentialed with Medicare, Medicaid and health plans PPH is actively contracted with (Senior Care Plus, Prominence, OPTUM,) or has the ability to be enrolled with any health plans.
- Minimum two years experience in specialty care area (Geriatrics) in private physician office, clinic setting or community-based program preferred.
- Demonstrates clinical competence including strong communications skills and medical knowledge.
- Remains current in licensure according to state regulation.
- Positively represents the practice by communicating to all visitors, patients and staff in an enthusiastic, friendly and professional manner.
- Provides a positive environment and experience with patients
- Actively seeks out methods to support the practice, staff, patients and physicians
- Participates in Continuing Medical Education as required by their governing board

Travel is required for this role. You must have the ability to drive between partner facilities regularly.
