Care Navigator - Victoria, BC
Status: Permanent Full-Time
Location: Remote to start, transitioning to in-office in Victoria, BC.

Role Overview
The Care Navigator plays a key role in supporting LifestyleRx patients by helping them access and benefit from allied healthcare services, including dietetics, physiotherapy, and clinical counselling.
This role sits at the intersection of patient support and service navigation. The primary focus is ensuring patients are informed, supported, and guided toward services that are clinically appropriate and beneficial to their care journey.
Care Navigators proactively engage with patients (primarily on the phone and over email) who have confirmed insurance coverage, respond to inbound inquiries, and follow up on physician-recommended services. All interactions are grounded in LifestyleRx’s core values: empathy, efficacy, and evidence.
Key Responsibilities
Patient Outreach & Engagement
Monitor and identify patients with confirmed extended health benefits for allied health services
Proactively notify patients of their available coverage and how it can be used within LifestyleRx
Conduct periodic follow-ups with patients to assess progress, answer questions, and provide guidance on available services
Support patients in understanding how allied health services integrate into their overall care plan
Service Navigation
Guide patients through available allied healthcare offerings (dietetics, physiotherapy, clinical counselling)
Help patients to assess whether services are appropriate based on their needs and goals
Support patients in enrolling in services only where there is a clear benefit to the patient
Ensure all recommendations are aligned with clinical guidance and evidence-based care
Inbound Support & Ticket Follow-up
Respond to patient inquiries received through the support inbox in a timely and professional manner
Follow up on service recommendation tickets submitted by LifestyleRx physicians
Coordinate with internal teams to ensure patient questions and concerns are resolved efficiently
Maintain clear and accurate documentation of all patient interactions
Collaboration & Care Coordination
Work closely with Patient Success, physicians, and allied health practitioners
Ensure continuity of care by reinforcing physician recommendations and care plans
Escalate clinical or complex concerns to the appropriate care team members

Qualifications & Experience:
Experience in patient support, care coordination, or inside sales (healthcare or wellness preferred)
Strong communication skills with the ability to build trust and rapport quickly
Ability to balance proactive outreach with sensitivity to patient needs
Comfortable working with CRM/ticketing systems and managing follow-ups
Understanding of extended health benefits (asset)
Interest in lifestyle medicine and preventative care
Thrives in a fast-moving environment where priorities shift and the playbook is still being written
Self-starter with an ownership mentality
Resourceful and scrappy: comfortable building process where none exists and iterating as you go

Pay Range
$70,000 – 80,000 per year, commensurate with experience

Schedule: 5 days a week (Full-Time 40 hours a week).
